| This release is executed on ____th day of ________ (month), 2008, in Phoenix, |
| Arizona, by ________________________________, parent or guardian or responsible |
| person, on behalf of ________________________________ (a minor child) referred to |
| as releaser to Joel F Johnson, Valley Chess, Borders Bookstore, Phoenix, Arizona, and |
| all parties associated with Valley Chess, hereinafter referred to as releasee. |

| In consideration for the participation of my child in the weekly meetings at Site |
| Sponsor during the calendar year of 2008, I as parent or guardian or responsible |
| person, voluntary release releasee from any and all injuries or damages to the minor |
| child and remain at all times responsible for the care, custody, control, safety and well |
| being of the minor child. In addition, I as parent or guardian or responsible person, will |
| remain present during the entire duration of weekly meeting while minor child is still |
| present. |

| This release contains the entire agreement between the parties hereto and the terms of this release are contractual and not a mere recital. |

| Releasor furthur states that he/she has read the foregoing release, and knows and understands the contents thereof and signs this release as his/her own free act. |
